File a Food Safety Complaint
Note: We do not investigate customer service complaints such as rude staff, slow service time, or lost reservations. Please report these incidents to the establishment’s management or corporation.
Grocery stores, gas stations, convenience stores, etc.
For general complaints regarding a grocery store, convenience store, party store, gas station, or processing facility, please contact the Michigan Department of Agriculture and Rural Development (MDARD).

Restaurants, bars, coffee shops, etc.
For complaints regarding a restaurant, bar, coffee shop, school, catering facility, or other facility serving food directly to the public in the City of Detroit* please call (313) 876-0135 or complete an online complaint form. Please be prepared to provide the following information:
- Your name, address, and phone number (this information will not be disclosed to the food establishment)
- Name of the restaurant or other food establishment
- Location/address of the establishment
- Date of incident
- Nature of the complaint
Foodborne illness complaints
If you believe you became ill from food you ate at a restaurant, grocery store, or other food establishment in the City of Detroit* please call (313) 876-0135 or complete an online complaint form. Please be prepared to provide the following information:
- Your name, address, and phone number (this information will not be disclosed to the food establishment)
- Name of the restaurant or other food establishment
- Location/address of the establishment
- Date and time of the suspect meal
- Specific information about the food(s) you believe made you sick
- Your three day food eating history
- What your symptoms are and when they began
